Someone calls with a real medical emergency — what does the AI do?

It doesn't play nurse: facing signs suggesting a medical emergency — chest pain, distress, immediate danger — the response is constant: direct the caller to emergency services and close cleanly. It's written into the base script and it's not negotiable, any more than the rule of never giving medical advice. The AI diagnoses nothing, offers no medical reassurance and doesn't say "wait for the callback". That reflex protects the caller, and incidentally you: an assistant improvising on health matters would be a danger, not a service.
